About the Program
The Extended Care and Rehabilitation Program at Louis Stokes Cleveland VA Medical Center is composed of four areas —
- Physical Medicine and Rehabilitation
- Extended Care Center
- Home and Community Care
- Geriatric Research Education and Clinical Center (GRECC)
Services Offered
Physical Medicine and Rehabilitation (PM&R)
PM&R services include —
- occupational therapy
- physical therapy
- speech therapy
- kinesiotherapy
- specialized services to veterans in need of rehabilitation following amputation, stroke, traumatic brain injury, and spinal cord injury
Extended Care Center (ECC)
The ECC is a hospital-based transitional care unit. ECC maintains an average length of stay of 32 days. Most patients require extended rehabilitation and care following surgery and/or lengthy hospitalizations before returning to independent living.
Home and Community Care (HCC)
HCC consists of programs that manage care in veterans homes and community settings. Services include —
- home-based primary care
- home telehealth care
- adult day health care
- community nursing home care
- skilled and unskilled home care
- home hospice care
Geriatric Research Education and Clinical Center (GRECC)
Geriatric Research, Education and Clinical Centers serve as resources in the latest in aging research, education and clinical care for the aging veteran.
GRECCs are "Centers of Excellence" that are designed to advance and integrate the latest research, education and clinical achievements to improve the quality of care to the aged.
Through research, we are working to develop ways to improve the quality of the life of our veterans as they age or if they are suffering from an injury.
We are researching new ways to improve everything we are affected with as we age.
Through education, we want to provide the latest information and training to our healthcare staff to improve the care we give our elderly veterans.
Through clinical practice, we are working to develop geriatric management programs and treatments to help veterans live more comfortably, relieves symptoms and improve the patient's quality of life.
The focus of the Cleveland GRECC is:
- Applied Clinical: Drug resistance in treatment of infection in the elderly
- Health Services: Aging and cancer care
- Rehabilitation: Effects of functional electric stimulation in stroke patients
