Patient Information

Patient Rights

Throughout the Cleveland VA Medical Center, the Patient and Nursing Home Resident Rights and Responsibilities Posters are displayed either in each building lobby area or on the patient care wards. Additionally, brochures or hand held versions of the poster are available from any of our Patient Representatives. Patient Representatives also distribute the brochures weekly on the psychiatric wards and at Nursing Home's Friends and Family meetings.

If you have a compliment, suggestion or concern regarding your care, first speak with your treatment team. Your treatment team includes your doctor, nurse, social worker, dietitian, pharmacist, chaplain, therapist and other professionals associated with your medical care. If you feel your concerns are not being addressed by your treatment team, you may contact any of our VA Patient Representatives.

A Patient Representative is an employee who is specifically designated at each medical center facility to manage the feedback received from veterans, family members and friends. The Patient Representative works directly with management and employees to facilitate resolutions. You may contact one of our Patient Representatives or the VA National Patient Advocate Website¹.
