About the Louis Stokes Cleveland VA Medical Center

The Cleveland VA Medical Center Center provides both inpatient and outpatient health care services at two facilities located in Cleveland (Wade Park) and Brecksville, Ohio. There are also 12 Community Based Outpatient Clinics with Northeast Ohio.

The Wade Park Campus is classified as a Level 1 (the most complex) type of medical facility because of the range of available services. It is a teaching hospital with full service patient care, as well as education & research center.

The Brecksville Campus includes extended care rehabilitation, general nursing home, center for psycho-geriatric care and a domiciliary for the homeless. The campus hosts a training facility of the VA’s Employee Education System and National Training Center.

  12. We take responsibility for providing seamless coordination of veterans care within VA offices, as well as non-VA facilities and organizations.

  14. We provide written and oral information and education about veterans health care that all veterans, their families and significant others will understand.

  15. Family Involvement
  16. We will provide the opportunity to involve veterans families and significant others in their care when appropriate.

  17. Transition
  18. We will provide a smooth transition between veterans inpatient and outpatient care.

  19. Specialty Care
  20. We provide the veterans with timely and convenient access and referral to necessary specialist health care.

  21. Pharmacy Service
  22. We provide veterans with timely and convenient access to pharmacy services both at the facility and through the Consolidated Mail Outpatient Pharmacy.
